Within a regional healthcare system, patients move through a network of providers, hospitals, general practitioners, emergency care, and specialised services. These flows are complex, interconnected, and constantly changing.Yet understanding them is essential for coordination, decision-making, and improving care.
Seeing the system as a whole
The challenge was to create an overview that makes these relationships visible. Not just showing movement, but structuring the system in a way that remains clear despite its complexity. A Sankey diagram forms the foundation, where the thickness of each flow represents the number of patients.This makes it possible to understand the system at a glance, revealing how patients move through the network and where the largest flows occur.
